Key Responsibilities

• Provide physical therapy services across a four-county rural service area, traveling to patient homes and surrounding communities
• Manage a caseload equivalent to approximately 4-6 patient visits daily using a point-based productivity system
• Complete OASIS Assessments and manage Start of Care cases
• Document time and visits using a facility-provided tablet
• Treat common cases including elderly falls and fractures

• Active Indiana Physical Therapist (PT) license - must be in hand at time of submission
• Prior home health experience required
• Home Care Home Base (HCHB) experience required
• Ability to perform OASIS Assessments and Start of Care evaluations
• Valid driver's license and reliable transportation for travel across a rural multi-county service area

• Experience treating geriatric patients, particularly those with fall-related injuries or fractures
• Familiarity with point-based productivity systems in a home health setting
• Comfort working independently in a rural, community-based environment
